A possible injury

  1. Tell the coach or Injury Liaison (IL) right away.
  2. The IL may use the King-Devick assessment and a symptom checklist.
  3. ILs do not diagnose; if there is concern, the precaution is to pull the player from play.

KNOW THE SIGNS

Symptoms can look different for every player.

Symptoms may be physical, cognitive, emotional, or related to sleep. They can change over time. Review all four categories below.

Physical

  • Headache
  • Nausea or vomiting
  • Fatigue
  • Dizziness or balance problems
  • Light or noise sensitivity

Thinking

  • Feeling foggy or slowed down
  • Trouble remembering
  • Difficulty concentrating
  • Visual problems

Emotional

  • Irritability
  • Anxiety
  • More emotional than usual
  • Sadness or nervousness

Sleep

  • Sleeping more or less
  • Drowsiness
  • Trouble falling asleep

Do not wait for a regular appointment if a danger sign appears.

A clean path from concern to return.

    Possible injury

    Tell the coach or Injury Liaison immediately.

    Assess & pause play

    Signs, symptoms, or a King-Devick concern mean the player comes out.

  3. 3
    Medical evaluation

    Parents arrange care with a concussion-experienced clinician.

  4. 4
    Documented return

    Submit the signed USA Hockey form before practice or games.

For a player removed from play for suspected concussion, a signed USA Hockey return-to-play form is required before resuming practices or games.
